The China Center of Adoption Affairs (CCAA)¡¯s requirements
for the content and format of post placement reports are as
follows:
Section I: Page One of the Post Placement Report
1. Name of the Adoption Agency: The name of the agency that
submitted the adoption dossier to the CCAA
¡¡¡¡2. Information on the Adopted Child:
¡¡¡¡¢ÙTravel Notice Number: (2004) MG-XXXX-XX-XXXX
¡¡¡¡¢ÚChinese Name: Chinese name at the time of adoption (Written
in Chinese characters or if more convenient, attach copy of
Travel Notice)
¡¡¡¡¢ÛForeign Name: New name as changed after the adoption
¡¡¡¡¢ÜDate of Birth: Adopted child¡¯s date of birth in China
¡¡¡¡¢ÝAdoption Date: Date of adoption registration in China
¡¡¡¡¢ÞName of the Social Welfare Organization: Name of the Chinese
welfare institute placing the child for adoption
¡¡¡¡¢ßHealth Status at Time of Adoption: Adopted child¡¯s health
status at the time the adoptive parents completed adoption registration
in China, if special needs, please specify the medical condition

¡¡¡¡¢ÛReport Date: Date the social worker completed the report
¡¡¡¡¢ÜReport Number: First or second post placement report
¡¡¡¡Section II: Report Text
1. Citizenship Situation: Status of the current procedures
to establish citizenship and estimated time of completion.
¡¡¡¡2. Attachment & Bonding Situation: Status of the adopted
child¡¯s blending with his/her parents, siblings, family, and
community. Give particular details if the family has adopted
a child over 3.
¡¡¡¡3. Health Situation: Adopted child¡¯s health status, immunizations,
and medical treatment situation, etc. Give particular details
on medical treatment and health status if the family has adopted
a special needs child.
¡¡¡¡4. Physical Development: Adopted child¡¯s height, weight, and
head circumference at the time of adoption, at the last report,
and currently, fine motor skill ability, and basic motor skill
ability.
¡¡¡¡5. Routine Activities: Adopted child¡¯s diet, playtime, and
sleep schedule.
¡¡¡¡6. Mental Development: Adopted child¡¯s mental development
and language ability.
¡¡¡¡7. Character Development: Adopted child¡¯s psychology and personality
characteristics.
¡¡¡¡8. Education Situation: Adopted child¡¯s learning at home,
kindergarten, and school and the method of education in the
home.
¡¡¡¡9. Family¡¯s Impression: Understanding and view toward the
adoption of the adoptive parents, other adults in the home,
and other relatives.
¡¡¡¡10. Major Changes in the Adoptive Family: Major changes in
the adoptive family since the home study report including changes
in marital status, employment, residence, or serious health
problems.
¡¡¡¡11. Other content the social worker believes should be included.
¡¡¡¡12. Social Worker¡¯s General Evaluation: Social worker¡¯s recommendation
of the adoptive family and general evaluation regarding the
success of the adoption.
Section III: Proof of Citizenship
If one of the following situations applies, when submitting
the post placement report, proof of citizenship for the adopted
child must also be included, such as: a copy of the child¡¯s
ID, a copy of the household register book, or a copy of the
adopted child¡¯s passport (in the adopting country), etc.
1. After the adopted child enters the adopting country, the
adopting country does not have a legal basis for automatically
granting citizenship;
¡¡¡¡2. The adopted child resides for more than one year in a country
other than the country of citizenship; or
¡¡¡¡3. A two-parent family jointly submitted the adoption dossier
to adopt the child, but only one parent traveled to China to
complete the adoption registration.
Section IV: Pictures
At the time the post placement report is submitted, four pictures
(more are not necessary) of the adopted child¡¯s life must also
be provided. The pictures must include a group photo of the
adopted child with the adoptive parents. Pictures must be mounted
two to one page of A4 size paper, and a brief description must
be provided below each picture.
¡¡¡¡*Other issues in the post placement reports:
1. Post placement reports must be evenly printed on A4 size
paper and may not be handwritten.
¡¡¡¡2. Post placement reports should be bound in the following
order: Chinese translation, original report, proof of citizenship,
pictures.
¡¡¡¡3. At the time the post placement reports are submitted, attach
two copies of a detailed list of the group¡¯s reports, listing
the names of all the adopted children whose reports are submitted
and their Travel Notice numbers. Upon receipt, we will check
the list against the dossier for errors and then sign the list.
One copy will be returned to the adoption agency.
¡¡¡¡4. Post placement reports must be mailed in their own envelope
to CCAA Archives.
