1. The basic content of the first page should be complete.
(1) Provide the document number (the number in the upper right-hand corner of the ¡°Notice of Coming to China for Adoption¡±) and the child¡¯s original Chinese name. If it is difficult to enter the child¡¯s name in Chinese characters, attach a copy of the ¡°Notice of Coming to China for Adoption¡± at the end of the report.
(2) Avoid errors. Mistakes in the child¡¯s name, the adopters¡¯ name(s), or the archive file and document number create difficulty for our registration and log-in staff. It is hoped that adoption organizations will properly preserve relevant information on the adopters (such as a copy of the ¡°Notice of Coming to China for Adoption¡±) so as to faithfully check against the relevant content of post placement reports.
(3) A Chinese translation should be placed at the front.
2. The report content cannot be overly simple and must not omit items.
The entire report should contain narration as well as a conclusion. The content of the two reports cannot be duplicated. American adoptive couples, where one party does not travel to China to carry out the adoption registration, must undergo court validation upon returning home. Families in this situation shall attach a copy of the court validation document when submitting post placement reports.
3. Requirements on submitting attached photographs
The CCAA requires no fewer than four photos. Photos must be mounted on white A4 standard paper, and below each photo must be a short explanation (may be written on the white paper), leaving space for a Chinese translation. The photos should be of good quality and digital photos should be printed on special photo paper.
4. There should be no large discrepancy between the time a post placement report is completed and the time it is submitted. It is hoped that each adoption organization will promptly dispatch a social worker to complete a home visit and compose a post placement report once an adoptive family¡¯s adoption is complete, according to the timing of the CCAA¡¯s regulations.
